‘Ohana Health Plan, a Centene (NYSE: CNC) company, has partnered with Hope Services Hawaii to launch a first-of-its-kind residential program for integrated clinical care in Hilo, Hawaii. The program features a six-bed facility exclusively for ‘Ohana members experiencing homelessness with complex behavioral and medical needs.

The program provides comprehensive care including behavioral health case management, licensed BH clinician services, APRN medication management, and housing case managers. Members receive integrated, sustained clinical care and case management during an average one-year stay. The facility is located on County of Hawaii property, leased to Hope Services.

This innovative model aims to reduce emergency room visits and inpatient care expenses while improving overall health outcomes by providing concentrated access to care for vulnerable members.

Positive
  • First-of-its-kind partnership addressing critical housing and healthcare needs
  • Expected reduction in emergency room and inpatient care expenses
  • Comprehensive integrated care model with multiple specialized services
Negative
  • None.

The launch of this residential program represents a strategic investment in preventive healthcare and cost management. The program’s focus on high-acuity homeless members with complex behavioral health needs addresses a important gap in Hawaii’s healthcare infrastructure. By providing integrated clinical care and stable housing, the initiative targets the intersection of two critical social determinants of health. The program’s six-bed facility may seem modest, but its potential impact on healthcare costs could be substantial.

The financial implications are particularly noteworthy. By reducing emergency room visits and inpatient care expenses through preventive and consistent care, this program could yield significant cost savings for Centene Data shows that homeless individuals typically incur 3-4 times higher healthcare costs compared to housed populations, primarily due to frequent emergency department utilization. This integrated care model, combining housing with comprehensive medical services, has demonstrated cost reductions of up to 40% in similar programs nationwide.

The partnership structure between a managed care organization and a housing agency creates a scalable model that could be replicated across other Centene markets, potentially establishing a new standard for addressing complex healthcare needs in vulnerable populations.

This initiative marks a significant evolution in addressing the complex interplay between homelessness and healthcare delivery. The program’s innovative feature lies in its exclusivity for ‘Ohana Health Plan members, creating a direct pathway from health insurance to housing stability. The one-year average stay duration is particularly well-conceived, providing sufficient time for stabilization while maintaining program throughput.

The multi-disciplinary approach, incorporating behavioral health clinicians, APRNs and housing case managers, addresses the full spectrum of needs that often trap individuals in cycles of homelessness and poor health. The program’s design shows sophisticated understanding of how housing instability impacts healthcare utilization and outcomes. By integrating clinical care with housing support, it creates a more effective intervention than traditional scattered-site housing programs or standalone medical services.

First-of-its-kind health plan and housing agency partnership in Hawaii offers transitional housing and structured support for its most vulnerable members experiencing homelessness

HILO, Hawaii, Jan. 14, 2025 /PRNewswire/ — ‘Ohana Health Plan, a leading provider of government-sponsored managed care services in Hawaii and a Centene Corporation company (NYSE: CNC), in partnership with Hope Services Hawaii, Hawaii Island’s largest nonprofit homeless services provider, held a grand opening and blessing for a brand-new residential program for integrated clinical care. The program addresses a critical gap in housing support for ‘Ohana’s highest acuity members experiencing homelessness, who also have behavioral health needs often combined with medical needs that are too complex for traditional subsidized housing.

The six-bed, Hilo-based home is reserved exclusively for ‘Ohana Health Plan members and provides integrated, sustained clinical care and case management for an average one-year stay. The property is owned by the County of Hawaii, which has an existing lease with Hope Services to provide emergency shelter for individuals experiencing homelessness.

The treatment-based residential program provides:

  • Behavioral health (BH) case management
  • Licensed BH clinician for therapy and psychosocial rehabilitation groups
  • Advanced Practice Registered Nurse (APRN) for medication management
  • Housing case managers
  • Multi-disciplinary team working with health coordination for comprehensive care

This model of concentrated access to care is expected to significantly reduce sudden and serious health problems that require immediate attention, thereby reducing acute emergency room or inpatient care expenses, and improving overall behavioral and physical health outcomes.

“At ‘Ohana Health Plan, we know that housing is healthcare, and we are committed to addressing housing through healthcare,” said Scott Sivik, plan president and chief executive officer of ‘Ohana Health Plan. “Our integrated care hub extends the continuum of care by allowing our most vulnerable members to recover and receive care in a safe environment. We couldn’t have asked for a better partner in Hope Services Hawaii to help us further our mission of transforming the health of the communities we serve, one person at a time.”

“Everyone deserves a safe place to call home, and this partnership with ‘Ohana Health Plan is another step toward that vision,” said Brandee Menino, chief executive officer of Hope Services Hawaii. “We also want to mahalo the County of Hawaii for their partnership, which is an invaluable resource for us to be able to make this happen. This innovative program represents what’s possible when we come together as a community to care for those who need it most. Together we’re giving our most vulnerable neighbors a safe place to heal, rebuild, and find hope for the future.”
